Hyperacute Epidural Hematoma Requiring Surgical Decompression after Epidural Adhesiolysis

Abstract
Adhesions in the epidural space can prevent the flow of medication to a target area. Epidural adhesiolysis can improve the delivery of medication to the affected area, potentially improving the therapeutic efficacy of the injected medications. We report a case of hyperacute epidural hematoma occurring immediately after a percutaneous epidural adhesiolysis procedure with NaviCath in patient without coagulopathy history. Within 10 minutes after procedure, the patient complained of severe back pain and back pain was aggravated with time. An emergency operation was performed 3.5 hours after the initial symptoms became evident. Immediately after the operation, the patient had regained sensation and showed marked improvement in pain. This case reveals that even though there are no neurologic symptoms, a patient¡¯s complaint such as back pain immediately after a procedure must be taken seriously and responded to with careful evaluation.
